The covering page on the file indicates the areas of concern to consider when providing your medical opinion, but we request that you read the file in its entirety before commencing your report.

PLEASE QUOTE ALL DOCUMENT NUMBERS FROM THE FILE WHEN REFERENCING THEM IN YOUR REPORT.
