Thank you for accepting this file for permanent impairment assessment.

The covering page on the file indicates the areas of concern to consider when providing your assessment of impairment, if any, but we request that you read the file in its entirety to determine if any other injuries warrant a permanent impairment rating. Please respond in point format with the identified injury followed by the permanent impairment percentage that you attribute to it. We will format your observations appropriately. Do not concern yourself with combining values, as we can do that in house. SGI has a specific format that we can create from the individual figures you provide.

PLEASE QUOTE ALL DOCUMENT NUMBERS FROM THE FILE AND ALL SECTIONS OF THE REGULATIONS THAT PERTAIN TO YOUR REPORT.

Most importantly, SGI is requesting a return of the file, which represents their originals, as soon as possible. Please do not use regular Canada mail, but either return the files to the office or let us know when we can pick-up the files and we will send a courier.

SGI specifically requests that the file not be marked up in any way, as it represents their original documents.
